Saturdays, Sundays, national holidays, anniversary of foundation (November 1st), year-end and New Year holidays (December 29th to January 3rd)

First visit

Please fill in the required items on the initial medical examination application form and apply at the first window.

Revisit

If you are going to the department within a year, please apply at the automatic return service machine.
If you are not able to apply, please contact the first window.

Other

  • If you do not have a referral letter [medical care information provision form] (*excluding referral letters from osteopath, orthopedic clinic and acupuncture), you will need to pay a separate “selective medical treatment fee (first visit 5,000 yen (excluding tax): own cost)”. However, the following cases are excluded.
    ・If you are seen as requiring life support.
    ・Even if it is the first department to receive medical examination, it is still being treated in another department of this hospital.
    ・When various public expenses are received for a specific disease or disorder.
    ・If you are eligible for medical assistance under the Welfare Act.
  • Our hospital checks the insurance card once a month.
  • Please note that the order may change depending on the symptoms.
  • Our hospital promotes referrals (reverse referrals) to local medical institutions.
